Interactive Forms

CLSMF has created interactive forms that you can use to create official letters and certain court filings. 

Use the sample letter Debt Collector Response if you want to tell a debt collector to stop contacting you. Read and follow these instructions to complete the letter.

Eviction Answer and Related Motions – use this interview to create your response to a Complaint for Eviction and related motions you will need.

Change of Address with the Court – use this form to provide the Court with your current address while a case is still pending.

Temporary Custody of Minor Children – use these forms to create a petition to be filed with the Court to seek custody of a minor relative who lives with you.
